Eligibility

Michigan Choice Waiver Program Eligibility
  • 18 years of age or older with a disability that would otherwise require nursing home care or over age 65 in need of nursing home care
  • Have an individual monthly income of $2,022 or less
  • Live in Livingston, Macomb, Monroe, Oakland or St. Clair or Washtenaw counties
  • Be Medicaid eligible
